Weight ManagementWeight Management Program Introduction: We offer a Medical Weight Management program that is individualized and customized for your specific needs. We utilize low calorie Optifast meal replacement formula as well as teaching you conscious eating techniques. Our program involves close physician supervision, individual and group counseling sessions, hormonal optimization, and nutrition education to help you achieve rapid weight loss and long term maintenance. This customized, comprehensive plan sets us apart from other commercial plans by offering direct physician supervision, personalized attention, and focusing on your long-term success. Pricing and how to get started: We required an initial consultation for anyone seriously considering the program. The initial consult is with Marcelle Olsen, MFT and Dr. Chen. After you have been evaluated by each of them, then you will meet with both Marcelle and Dr. Chen to receive a customized plan. The consult is 200 dollars payable at Dr. Chen’s office, at which time, you will be given the materials necessary to complete the consultation process. Once you received the customized plan, you can then decide whether to participate in the plan. The cost of the program will be different for everyone depending on their starting weight and their goal. It is approximately 395 dollars plus cost of the Optifast formula per month. ( please note that Optifast may not be prescribed for some patients) Scheduling appointments and Ordering your Optifast: Initial Consultation: 1. You will come to RAM ( 2451 Buhne St, Eureka ) to pay fee of $200 to get a packet and schedule an evaluation with Dr. Chen in about 2 weeks. 2. You will call Marcelle Olsen to schedule an appointment. ( phone and office address will be in the packet given to you, you will need to complete and bring this packet to your appointment with Marcelle) 3. After both evaluations and the required lab and eletrocardiogram testing ( may be covered by your health insurance )are done, you will meet with M & C to review your plan. This appointment will be on Thursday evenings between 5:15-6 pm. Program Start: Once you decide to start the program, you will need to have your Optifast order placed, please note it will take 7-10 days before you will receive your order, it will be shipped to your house. You can place the order with our staff at RAM., at which time, the monthly fee will be due, and all remaining program consent and paperwork will be completed at that time. Follow up scheduling: You will follow up with Dr. Chen every other Tuesday evening at 5 pm at RAM, and with Marcelle for small group session every Tuesday evening at 630 pm, In addition, you will schedule an individual session with Marcelle once a month. Once you reached the maintenance phase, you can still attend the Tuesday evening meeting with Marcelle at a fee of $20/session. You may also schedule further follow up appointment and purchase additional Optifast product with Dr. Chen at RAM. |
